How to Complete Patient Portal Forms on your Cell Phone

  2. Sign in with your 91传媒 Portal credentials.

  3. Click the menu bar (grey bar with three blue lines).

  4. Click on “Forms”.

  5. Scroll down to “91传媒 Patient Forms” and complete the following forms: 91传媒 Medical History, 91传媒 Privacy Policy Acknowledgement, 91传媒 Release Authorization, and all other forms related to your visit (91传媒 Physical Form for Physicals, STI Test Consent Form for Sexual Health/UTI Visits, TB Questionnaire for TB Skin Tests, Women’s Health Form for Women’s Health Visits, etc.)

  6. Go to “Messages” for Class Excuses. Go to “Statements” for receipts from your visit.
